Below is a copy of the Roofing Requirement form that must be completed for any roofing or re-roofing work in Telford Borough, along with a Telford Borough Building Permit Application:

Roofing Requirement Form

A copy of the contractor's Certificate of Insurance that includes liability insurance and workers comp insurance must also be submitted along with the permit application and roofing requirements form.  If the contractor does not have workers comp insurance, the following waiver must be completed and submitted along with the liability insurance: Telford Borough Workers Compensation Affidavit.

The contractor's PA Contractor's License number must also be filled in on the permit application.
